Un&nbsp;agent de contraste id\u00e9al doit avoir une efficacit\u00e9 maximale (att\u00e9nuation maximale des&nbsp;rayons&nbsp;X) et une&nbsp;innocuit\u00e9 maximale (ne produire aucun effet ind\u00e9sirable).&nbsp; Dans la pratique r\u00e9elle, l&rsquo;innocuit\u00e9 maximale signifie qu&rsquo;un agent de contraste devrait avoir des&nbsp;effets toxiques ou ind\u00e9sirables minimes sur les&nbsp;fonctions physiologiques et pr\u00e9server le&nbsp;confort du patient. Le radiologue clinicien su\u00e9dois Torsten Alm\u00e9n a \u00e9t\u00e9 l&rsquo;un des champions de ce d\u00e9fi.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><img decoding=\"async\" src=\"https:\/\/car.ca\/wp-content\/uploads\/2024\/08\/gehc1-2.png\" alt=\"Cliquez sur l'image pour regarder un court extrait sur l'\u00e9volution des produits de contraste\" title=\"gehc1 2\"\/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<p>M\u00e9decin praticien dans les&nbsp;ann\u00e9es 1960, le Dr&nbsp;Alm\u00e9n s&rsquo;inqui\u00e9tait de la douleur ressentie par ses patients lorsqu&rsquo;il leur injectait l&rsquo;agent de contraste \u00e0 haute osmolalit\u00e9 (HOCM) disponible. Il a un jour remarqu\u00e9 que lorsqu&rsquo;il nageait dans l&rsquo;eau sal\u00e9e le long de la c\u00f4te ouest de la Su\u00e8de, ses yeux br\u00fblaient; en revanche, lorsqu&rsquo;il nageait dans la mer Baltique, qui est moins sal\u00e9e, il ne sentait rien. Le Dr&nbsp;Alm\u00e9n a alors d\u00e9velopp\u00e9 une th\u00e9orie&nbsp;: la douleur que ses patients ressentaient \u00e9tait due \u00e0 la haute osmolalit\u00e9 des agents de contraste<sup>1<\/sup>.<\/p>\n\n\n\n<p>Inspir\u00e9, il a \u00e9mis l&rsquo;hypoth\u00e8se que l&rsquo;osmolalit\u00e9 des&nbsp;agents de contraste iod\u00e9s \u00e9tait la&nbsp;principale cause de leurs effets secondaires. Gr\u00e2ce \u00e0 sa formation en chimie organique, le Dr&nbsp;Alm\u00e9n a imagin\u00e9 une&nbsp;fa\u00e7on de r\u00e9duire l&rsquo;osmolalit\u00e9 des agents de contraste iod\u00e9s pour produire un agent de contraste \u00e0 basse osmolalit\u00e9 (LOCM) et, ensuite, un agent de contraste iso-osmolaire (IOCM)<sup>1<\/sup>.<\/p>\n\n\n\n<p>L&rsquo;osmolalit\u00e9 d&rsquo;un agent de contraste se d\u00e9termine par le nombre de particules osmotiquement actives pr\u00e9sentes lorsque l&rsquo;agent de contraste se dissout dans la solution. L&rsquo;osmolalit\u00e9 est exprim\u00e9e en termes de concentration de particules osmotiquement actives (milliosmoles) par unit\u00e9 de poids (kilogrammes) d&rsquo;eau, ou mOsm\/kg&nbsp;H2O. Plus le rapport entre le nombre d&rsquo;atomes d&rsquo;iode divis\u00e9 par le nombre de particules dans une solution est \u00e9lev\u00e9, plus l&rsquo;osmolalit\u00e9 est basse<sup>2<\/sup>.<\/p>\n\n\n\n<p>La forme de base d&rsquo;un&nbsp;agent de contraste pour radiographie est un anneau de benz\u00e8ne iod\u00e9 (comme le montre cette illustration du diatrizoate ci-dessous). G\u00e9n\u00e9ralement, 3&nbsp;atomes d&rsquo;iode sont lib\u00e9r\u00e9s avec chaque anneau de benz\u00e8ne d&rsquo;un&nbsp;agent de contraste. Les HOCM comme le diatrizoate sont ioniques, ce qui signifie qu&rsquo;ils se d\u00e9composent en deux particules osmotiquement actives (un anion charg\u00e9 n\u00e9gativement et un cation charg\u00e9 positivement) en solution. Par cons\u00e9quent, ils lib\u00e8rent 3&nbsp;atomes d&rsquo;iode pour chaque 2&nbsp;particules osmolaires (3:2). Cela donne un&nbsp;agent \u00e0 haute osmolalit\u00e9, jusqu&rsquo;\u00e0 8&nbsp;fois sup\u00e9rieure \u00e0 celle du sang<sup>3<\/sup>.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><img decoding=\"async\" src=\"https:\/\/car.ca\/wp-content\/uploads\/2024\/08\/gehc-fr2.png\" alt=\"gehc fr2\" title=\"gehc fr2\"\/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<p>En d\u00e9veloppant un&nbsp;agent de contraste non ionique, le Dr&nbsp;Alm\u00e9n \u00e9tait convaincu qu&rsquo;il \u00e9tait possible de r\u00e9duire l&rsquo;osmolalit\u00e9 de fa\u00e7on significative \u00e0 environ la moiti\u00e9 de celle d&rsquo;un&nbsp;HOCM. En solution, les&nbsp;agents non ioniques ne se d\u00e9composent pas en deux particules osmotiquement actives. Cela signifie qu&rsquo;ils peuvent lib\u00e9rer 3&nbsp;atomes d&rsquo;iode pour chaque particule osmolaire (3:1). Comme cela a pour r\u00e9sultat moins de mol\u00e9cules par kilogramme d&rsquo;eau qu&rsquo;un&nbsp;HOCM, l&rsquo;osmolalit\u00e9 d&rsquo;un LOCM, comme l&rsquo;iohexol, est r\u00e9duite<sup>3<\/sup>.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><img decoding=\"async\" src=\"https:\/\/car.ca\/wp-content\/uploads\/2024\/08\/gehc-fr3.png\" alt=\"gehc fr3\" title=\"gehc fr3\"\/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<p>Une mol\u00e9cule de contraste avec un seul anneau de benz\u00e8ne est appel\u00e9e un monom\u00e8re. En liant deux monom\u00e8res non ioniques pour former un&nbsp;dim\u00e8re, il est possible de lib\u00e9rer une plus grande quantit\u00e9 d&rsquo;iode avec moins d&rsquo;effet sur l&rsquo;osmolalit\u00e9. La synth\u00e8se de Visipaque (iodixanol), un dim\u00e8re non ionique, a concr\u00e9tis\u00e9 les id\u00e9es visionnaires du Dr&nbsp;Alm\u00e9n en mati\u00e8re d&rsquo;\u00e9volution de l&rsquo;osmolalit\u00e9 des agents de contraste iod\u00e9s. Visipaque est iso-osmolaire, ce qui signifie qu&rsquo;il a la m\u00eame osmolalit\u00e9 que le&nbsp;sang \u00e0 toutes les concentrations. Pour chaque particule osmolaire, Visipaque lib\u00e8re 6&nbsp;atomes d&rsquo;iode (6:1)<sup>3<\/sup>.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><img decoding=\"async\" src=\"https:\/\/car.ca\/wp-content\/uploads\/2024\/08\/gehc-fr4.png\" alt=\"gehc fr4\" title=\"gehc fr4\"\/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<p>Les agents de contraste iod\u00e9s ont beaucoup \u00e9volu\u00e9 depuis le d\u00e9veloppement des agents de premi\u00e8re g\u00e9n\u00e9ration, les&nbsp;HOCM, dans les ann\u00e9es 1950.
