Application for MAP Accreditation

The Canadian Association of Radiologists offers radiologists the opportunity for peer review and evaluation of their facility's staff qualifications, equipment performance, quality control and quality assurance programs, image quality, breast dose, and processor quality control. This voluntary program is administered by the Mammography Accreditation Program (MAP) of the Canadian Association of Radiologists (CAR).


The facility must first complete an initial application to provide information regarding the facility, equipment, personnel qualifications, quality control results and submit a copy of the most recent physicist report (survey must have been carried out within the last 8 months). No clinical or phantom images should be submitted at this time. If the facility meets the basic CAR requirements for accreditation, Part II of the application will be mailed out by CAR with the appropriate testing materials.
